In conjunction with AIM Academy ,  Eric's RX Shoppe is happy to partner to provide the Moderna & Pfizer COVID-19 Vaccine 4th Dose for ages 50+. Details of the clinic are below:

The vaccine schedule is as follows:​​

• 4th Dose: Friday, April 8th from 1:00-1:30PM

• Must be 50+ years of age and have received the Pfizer or Moderna COVID-19 vaccine at least 4 months prior to the date of this clinic. Please see the link for additional guidance from the CDC.

You will be directed to our booking system site by clicking the above and below links.

In the Primary Care Physician screen, please input the Physician's name and for the phone number, only use numbers (no dashes or spaces). Please make sure you receive an email confirmation with your appointment time, otherwise you do not have a confirmed appointment.

You will receive an access code at the email address you provide on the first screen. On the second screen, enter the verification code found in your email to be able to select an appointment time for both doses. 

​

Registration will close on Thursday, April 7th at 12:00PM.
