VACCINE CLINIC

In conjunction with AIM Academy, Eric's RX Shoppe is happy to partner to provide a vaccine clinic for ages 3+. Details of the event are below:

Date: Friday, October 6th from 10:00 AM - 12:00 PM

Where: AIM Academy - 1200 River Rd, Conshohocken, PA 19428

Vaccine Offered:

Flu Shots - Ages 3+ (High Dose for ages 65+)
COVID-19 Seasonal Vaccine (SpikeVax) - INSURANCE CARD REQUIRED (Pending Coverage/Availability)
- Moderna (SpikeVax) Vaccine Ages 12+ and Moderna Vaccine for Ages 3-11
- Currently, only limited supply of this vaccine is available. And the uninsured/underinsured program is not up and running. We are not able to provide any doses of the COVID-19 Vaccine to those without insurance and you could be subject to a copay or cost sharing. The vaccine cost $130 per dose

Please pre-register and be sure to fill out the insurance section of registration.

IF THIS IS FOR A MINOR, PLEASE REGISTER USING YOUR CHILD'S NAME, DOB, AND ALL OTHER INFORMATION!

If you are under 18, you must have either a parent/guardian present or have the proper consent form(s) printed and signed (NO EXCEPTIONS).

Registration will close on Tuesday, October 3rd.
