VACCINE CLINIC 

In conjunction with Eastern Center for Arts and Technology, Eric's RX Shoppe is happy to partner to provide a vaccine clinic.  Details of the event are below:

Date: Tuesday, October 22nd from 10:00 AM - 12:00 PM

Where: 3075 Terwood Rd, Willow Grove, PA, 19090

 

Vaccine Offered:​​​

• ​Flu Shots - Ages 5-64

• Flu Shots - Ages 65+ High Dose

• SpikeVax Moderna Covid Vaccine - Ages 12+ (must be a minimum of 2 months from prior dose)

Please preregister and be sure to complete the insurance section.

• IF THIS IS FOR A MINOR, PLEASE REGISTER USING YOUR CHILD'S NAME, DOB, AND ALL OTHER INFORMATION!

If you are under 18, you must have either a parent/guardian present or have the proper consent form(s) printed and signed (NO EXCEPTIONS).
