STAFF FLU VACCINE CLINIC

In conjunction with the Kendal Communities, Eric's RX Shoppe is happy to partner to provide a flu vaccine clinic.  Details of the event are below:

Date: Tuesday, September 19th, 2023

Time/Location: ( you may sign up for either location)

• 10am-12pm Kendal Longwood

• 1pm-3pm Crosslands / 3pm-4:45pm Culinary Staff

Vaccine Offered:​​​

• ​Flu Shots 

        (anyone 65+ years of age will receive the High Dose Flu Shot)​

​

Pre-registration is highly recommended, but walk-ins, are welcome.​

​

If you are under 18, you must have either a parent/guardian present or have the proper consent form(s) printed and signed (NO EXCEPTIONS). ​​

Please be sure to fill out the insurance section and/or upload a copy of your card.  If you do not have insurance, please select that option in the drop down.

Registration will close on September 13th at 12pm.
