STAFF VACCINE CLINIC 

In conjunction with Lower Moreland School District, Eric's RX Shoppe is happy to partner to provide a vaccine clinic.  Details of the event are below:

Date: Wednesday, September 27th from 2:30 PM - 5:30 PM

Where: Lower Moreland High School, 555 Red Lion Rd, Huntington Valley, PA, 19006 - GYM

 

Vaccine Offered:​​​

• ​Flu Shots - Ages 5-64 and High Dose for ages 65+

• Covid-19 Vaccine - Ages 12+ (pending insurance coverage)

If you are under 18, you must have either a parent/guardian present or have the proper consent form(s) printed and signed (NO EXCEPTIONS). ​​

• COVID/FLU MINOR CONSENT FORM

Please be sure to complete the insurance section of the registration.

​

Registration will close on September 24th.
