COMMUNITY VACCINE CLINIC 

In conjunction with Rep. Greg Scott/Simon's Heart Community Health Screening, Eric's RX Shoppe is happy to partner to provide a vaccine clinic.  Details of the event are below:

Date: Saturday, September 23rd from 10:00 AM - 12:00 PM

Where: Roosevelt Campus of the Norristown Area High School

              1161 Markley Street, Norristown, PA 19401

Vaccine Offered:​​​​

• Flu Vaccine

• Covid-19 Vaccine (pending approval from FDA/CDC)

Pre-registration is highly recommended, but walk-ins are welcome.​

• IF THIS IS FOR A MINOR, PLEASE REGISTER USING YOUR CHILD'S NAME, DOB, AND ALL OTHER INFORMATION!

If you are under 18, you must have either a parent/guardian present or have the proper consent form(s) printed and signed (NO EXCEPTIONS).

The insurance section of registration is not required, but completion is highly encouraged.
