PFIZER COVID-19 VACCINE CLINIC
AGES 3 YEARS AND UP 

 

In conjunction with Temple Sinai, Eric's RX Shoppe is happy to partner to provide the Pfizer COVID-19 Vaccine for ages 3 years and up.  Details of the event are below:

The vaccine schedule is as follows:​​

• Wednesday, July 6th from 3:00PM - 4:00PM

• Second Dose (if applicable),  Wednesday, July 27th from 3:00PM - 4:00PM

• Third Dose (if applicable),  Thursday, September 22nd from 3:00PM - 4:00PM

     (at Eric's RX Shoppe, 810 E. Welsh Rd, Horsham, PA 19044 NOT Temple Sinai)​

Where: Temple Sinai, Auditorium - 1401 Limekiln Pike, Dresher, PA 19025

 

Vaccine Offered:​

• Pfizer - Ages 3-4

1st Dose

2nd Dose (must be a minimum of 21 days from the 1st dose)

3rd Dose (must be a minimum of 57 days from the 2nd dose)

• Pfizer - Ages 5-11

1st and 2nd Dose (must be a minimum of 21 days from the 1st dose)

Boosters (must be a minimum of 5 months from the 2nd dose)

• Pfizer - Ages 12+

1st and 2nd Dose (must be a minimum of 21 days from the 1st dose)

Boosters (must be a minimum of 5 months from the 2nd dose)

• Pfizer - Ages 50+

2nd Boosters (must be a minimum of 4 months from the 3rd dose)

Registration is preferred, but walk-ins are welcome.

​

If you are under 18, you must have either a parent/legal guardian present.

​

A page will appear titled Insurance Information. This is not a required section; however, if you do provide the information, it asks for your physician telephone #. Please do not include the dashes (-) between the numbers.

PLEASE REGISTER USING YOUR CHILD'S NAME, DOB, AND ALL OTHER INFORMATION!

You will receive an access code at the email address you provide on the first screen. On the second screen, enter the verification code found in your email to be able to select an appointment time for both doses. 

​

Registration will close on July 5th @ 12:00PM
