FLU VACCINE CLINIC 

In conjunction with Turn 14, Eric's RX Shoppe is happy to partner to provide a vaccine clinic for ages 3+.  Details of the event are below:

Date: Tuesday, September 12th from 11 AM - 12 PM

Where:  100 Tournament Dr. Horsham, PA, 19044

 

Vaccine Offered:​​​

• ​Flu Shots - Ages 3+ (High Dose for ages 65+)

​

Pre-registration is highly recommended.

• IF THIS IS FOR A MINOR, PLEASE REGISTER USING YOUR CHILD'S NAME, DOB, AND ALL OTHER INFORMATION!

If you are under 18, you must have either a parent/guardian present or have the proper consent form(s) printed and signed (NO EXCEPTIONS). ​​

• FLU MINOR CONSENT FORM

The insurance section of registration is not required but completion is highly encouraged.

​

Registration will close on September 10th.
