FLU/COVID/RSV  VACCINE CLINIC 

In conjunction with AIM Academy, Eric's RX Shoppe is happy to partner to provide a vaccine clinic for ages 5+.  Details of the event are below:

• Flu HD Vaccine - Ages 65+

• RSV - Ages 50+

• SpikeVax by Moderna Covid 19 Vaccine - Ages 3-11​​

FOR ALL COVID VACCINES:
I understand the benefits of vaccination and possible side effects. I agree to receive the vaccine as part of a shared decision with my healthcare provider (pharmacist).

If you are under 18, you must have either a parent/guardian present or have the proper consent form(s) completed and emailed back to Lindsay Brown at lbrown@aimpa.org prior to the clinic (NO EXCEPTIONS). Minor students without a parent/guardian present or consent form WILL NOT be vaccinated.
