BACK TO SCHOOL VACCINE CLINIC

In conjunction with Cheltenham School District and The Montgomery County Office of Public Health, Eric's RX Shoppe is happy to partner to provide a vaccine clinic.  Details of the event are below:

• 10:30am - 11:30am - Cheltenham High School Students Only -500 Rices Mill Rd, Wyncote, PA 19095 (Nurses Suite)

• 11:45am - 12:45pm - Cedarbrook Middle School Students Only-300 Longfellow Rd, Wyncote, PA 19095 (auditorium)​​

Vaccine Requirements:​​

​FOR ATTENDANCE IN 7TH GRADE:

•    1 dose of tetanus, diphtheria, acellular pertussis (Tdap) on the first day of 7th grade.
•    1 dose of meningococcal conjugate vaccine (MCV) on the first day of 7th grade.

FOR ATTENDANCE IN 12TH GRADE:

• 1 dose of meningococcal conjugate vaccine (MCV) on the first day of 12th grade.

If one dose was given at 16 years of age or older, that shall count as the twelfth grade dose.

**Pre-registration is required. Insurance information must be included in the registration. If you do not have insurance, a dose will be provided by the county at no cost to you**

• IF THIS IS FOR A MINOR, PLEASE REGISTER USING YOUR CHILD'S NAME, DOB, AND ALL OTHER INFORMATION!

If you are under 18, you must have either a parent/guardian present or have the proper below consent form completed and printed.  The student must have the hard copy present in order to be vaccinated.

(NO EXCEPTIONS).
