In conjunction with Eastern Center for Arts and Technology, Eric's RX Shoppe is happy to partner to provide a vaccine clinic.  Details of the event are below:

• Flu Vaccine - Ages 3-64

• Flu Vaccine - Ages 65+ (High Dose)

• RSV  Vaccine - Ages 50+

• Pneumonia Vaccine - Ages 50+

• Shingles Vaccine (1st or 2nd Dose) - Ages 50+

• SpikeVax by Moderna Covid 19 Vaccine - Ages 3-11​​

• mNEXSPIKE by Moderna Covid 19 Vaccine - Ages - 12+​

FOR ALL COVID VACCINES:
I understand the benefits of vaccination and possible side effects. I agree to receive the vaccine as part of a shared decision with my healthcare provider (pharmacist).
