In conjunction with The Gateway School, Eric's RX Shoppe is happy to partner to provide a vaccine clinic.  Details of the event are below:

Date: Friday, October 16th from 8:00 AM-9:00AM

• Flu Vaccine - Ages under 65

• Flu HD Vaccine - Ages 65+​​

• mNEXSPIKE by Moderna Covid 19 Vaccine - Ages 12+​

FOR ALL COVID VACCINES:
I understand the benefits of vaccination and possible side effects. I agree to receive the vaccine as part of a shared decision with my healthcare provider (pharmacist).

Must be a minimum of 3 months since your last covid vaccine​
