In conjunction with the Lower Merion School District,  Eric's RX Shoppe is happy to partner to provide the Pfizer COVID-19 Bivalent  Boosters for ages 5+ and Flu Shots for anyone 3+.

Date: Friday, October 28th from 3 PM - 6 PM

Where: Lower Merion High School, Cafeteria (315 E Montgomery Ave, Ardmore, PA 19003)

 

Vaccine Offered:​​

Pfizer COVID-19 Variant Booster (BIVALENT)- Ages 12+​

Anyone who completed their primary series (Pfizer, Moderna, or J&J) can get a Pfizer-BioNTech COVID-19 Bivalent Booster. 

Must be 12+ years of age, and

Must be a minimum of 2 months from the completion of a primary series or last booster dose.

Pfizer COVID-19 Variant Booster (BIVALENT) - Ages 5-11 ​

Anyone who completed their primary series (Pfizer or Moderna) can get a Pfizer-BioNTech COVID-19 Bivalent Booster. 

Must be 5+ years of age, and

Must be a minimum of 2 months from the completion of a primary series or last booster dose.

Flu Shots - Ages 3+ (High Dose for ages 65+)

This clinic is for any current Lower Merion School District Students ages 5+.  Pre-registration is required.

If you are under 18, you must have either a parent/legal guardian present or have this CONSENT FORM 

printed and signed (NO EXCEPTIONS).
