In conjunction with Lower Moreland School District, Eric's RX Shoppe is happy to partner to provide a vaccine clinic.  Details of the event are below:

Date: Saturday, October 18th from 9:00AM - 11:00AM

Where: Lower Moreland High School, 545 Red Lion Rd, Huntington Valley, PA, 19006 - Auxiliary Gym

Vaccine Offered:​​​​

• Flu Vaccine - Ages 3-64

• Flu Vaccine - Ages 65+ (High Dose)

• SpikeVax by Moderna Covid 19 Vaccine - Ages 3-11​​

• mNEXSPIKE by Moderna Covid 19 Vaccine - Ages - 12+​

FOR ALL COVID VACCINES:

I understand the benefits of vaccination and possible side effects. I agree to receive the vaccine as part of a shared decision with my healthcare provider (pharmacist).

 

Please pre-register and be sure to fill out the insurance section of registration.

If you are under 18, you must have either a parent/guardian present or have the proper consent form(s) printed and signed (NO EXCEPTIONS).
