In conjunction with The Miquon School, Eric's RX Shoppe is happy to partner to provide a vaccine clinic.  Details of the event are below:

• Flu Vaccine - Ages 3-64

• Flu Vaccine - Ages 65+ (High Dose)

• SpikeVax by Moderna Covid-19 Vaccine - Ages 3-11​​

• mNEXSPIKE by Moderna Covid-19 Vaccine - Ages 12+​

FOR ALL COVID VACCINES:

I understand the benefits of vaccination and possible side effects. I agree to receive the vaccine as part of a shared decision with my healthcare provider (pharmacist).
