In conjunction with The Miquon School, Eric's RX Shoppe is happy to partner to provide a vaccine clinic.  Details of the event are below:

Date: Friday, February 23rd from 2:30 PM - 4:00 PM

Where: Miquon School - 2025 Harts Ln, Conshohocken, PA 19428

 

Vaccine Offered:​​​

• ​Influenza (Flu Vaccine) - Ages 3+ 

• Influenza (High Dose Flu Vaccine) - Ages 65+ 

• COVID-19 Seasonal Vaccine - INSURANCE CARD REQUIRED

  • Moderna Vaccine for Ages 3-11(must be a min. of 2 months from previous dose)

  • Moderna (Spikevax) Vaccine Ages 12+ (must be a min. of 2 months from previous dose)

Pre-registration is required.  Be sure to complete the insurance section of registration.

• ​IF THIS IS FOR A MINOR, PLEASE REGISTER USING YOUR CHILD'S NAME, DOB, AND ALL OTHER INFORMATION!

If you are under 18, you must have either a parent/guardian present or have the proper consent form(s) printed and signed (NO EXCEPTIONS).
