VACCINE CLINIC 

In conjunction with Montgomery County Senior Adult Activity Center, Eric's RX Shoppe is happy to partner to provide a vaccination clinic.  Details of the event are below:

Date: Friday, September 22nd from 10:00 AM - 12:00 PM

Where: 45 Forest Ave, Ambler, PA, 19002

 

Vaccine Offered:​​​

• ​Flu Shots - Ages 3+ and (High Dose for ages 65+)

• COVID-19 Vaccine (pending approval)

****Montco SAAC will provide a $10 gift card incentive to every recipient that is 60+ and/or disabled.****

​

Pre-registration is highly recommended.

• IF THIS IS FOR A MINOR, PLEASE REGISTER USING YOUR CHILD'S NAME, DOB, AND ALL OTHER INFORMATION!

If you are under 18, you must have either a parent/guardian present or have the proper consent form(s) printed and signed (NO EXCEPTIONS).
