STAFF COVID VACCINE CLINIC

In conjunction with the PPH, Eric's RX Shoppe is happy to partner to provide a flu vaccine clinic.  Details of the event are below:

Date: Thursday, October 26th from 10:30am-2:00pm

Location: 6401 Martins Mill Rd, Philadelphia, PA 19111

Vaccine Offered:​​​

• COVID-19 Seasonal Vaccine

  • Moderna (SpikeVax) Vaccine Ages 12+ (must be a min. of 2 months from previous dose)

  • Covid Vaccine cards do not need to be presented

  • The shot should be covered by insurance with no copay. We have confirmed coverage under the company policy issued for PPH Employees. If the shot is not covered, you may be responsible for the cost of the vaccine. If you do not have insurance, please register at www.covidaccess.com and you must bring the payment voucher to your appointment

Pre-registration is required.  Please be sure to complete the insurance section of registration.​

​

If you are under 18, you must have either a parent/guardian present or have the proper consent form(s) printed and signed (NO EXCEPTIONS). ​​
