STAFF COVID/FLU VACCINE CLINIC

In conjunction with the PPH, Eric's RX Shoppe is happy to partner to provide a flu vaccine clinic.  Details of the event are below:

Date: Wednesday, November 5th from 10:00am-12:00pm

Vaccine Offered:​​​​

• Flu Vaccine - Ages under 65

• Flu Vaccine - Ages 65+ (High Dose)​​

• mNEXSPIKE by Moderna Covid 19 Vaccine - Ages - 12+​

FOR ALL COVID VACCINES:
I understand the benefits of vaccination and possible side effects. I agree to receive the vaccine as part of a shared decision with my healthcare provider (pharmacist).

Must be a minimum of 3 months since your last covid vaccine​

Be sure to pre-register.  Please be sure to complete the insurance section of registration.​

If you are under 18, you must have either a parent/guardian present or have the proper consent form(s) printed and signed (NO EXCEPTIONS). ​​
