STAFF COVID/FLU VACCINE CLINIC

In conjunction with the PPH, Eric's RX Shoppe is happy to partner to provide a flu vaccine clinic.  Details of the event are below:

Date: Wednesday, October 30th from 10:00am-12:00pm

Location: 6401 Martins Mill Rd, Philadelphia, PA 19111

Vaccine Offered:​​​

• SpikeVax by Moderna COVID-19 Vaccine - Ages 12+​

• Flu Vaccine - Ages 5-64

• Flu Vaccine - Ages 65+ (High Dose)

Pre-registration is required.  Please be sure to complete the insurance section of registration.​

​

If you are under 18, you must have either a parent/guardian present or have the proper consent form(s) printed and signed (NO EXCEPTIONS). ​​
