Eric’s RX are pleased to partner with him to provide a vaccine clinic.  Details of the event are below:

Where: Neshaminy Activity Center-1842 Brownsville Rd, Feasterville-Trevose, PA

Vaccine Offered:​​​​

• Flu Vaccine - Ages 3-64

• Flu HD Vaccine - Ages 65+

• RSV Vaccine- Ages 50+​​

• mNEXSPIKE by Moderna Covid 19 Vaccine - Ages 12+​

FOR ALL COVID VACCINES:
I understand the benefits of vaccination and possible side effects. I agree to receive the vaccine as part of a shared decision with my healthcare provider (pharmacist).

Vaccines can be co-administered, but we will administer a maximum of 2 vaccines.​​

​

​Pre-registration is highly recommended, but walk-ins are welcome.

Be sure to complete the insurance section of registration.  Insurance verification and coverage is required for the RSV vaccine.
