In conjunction with Rep Munroe Senior Expo, Eric's RX Shoppe is happy to partner to provide a vaccine clinic.  Details of the event are below:

Vaccine Offered:​​​​

• Flu Vaccine - Ages under 65

• Flu Vaccine - Ages 65+ (High Dose)​​

• mNEXSPIKE by Moderna Covid 19 Vaccine - Ages - 12+​

FOR ALL COVID VACCINES:

I understand the benefits of vaccination and possible side effects. I agree to receive the vaccine as part of a shared decision with my healthcare provider (pharmacist).

Must be a minimum of 3 months since your last covid vaccine​
