In conjunction with the Rydal Park, Eric's RX Shoppe is happy to partner to provide a vaccine clinic.

FOR ALL COVID VACCINES:
I understand the benefits of vaccination and possible side effects. I agree to receive the vaccine as part of a shared decision with my healthcare provider (pharmacist).

Must be a minimum of 3 months since your last covid vaccine​

Be sure to pre-register.  Please be sure to complete the insurance section of registration.​
