SAY BOO TO THE FLU
VACCINE CLINIC 

In conjunction with Elmwood Park Zoo and Montgomery County Immunization Coalition, Eric's RX Shoppe is happy to partner to provide a vaccine clinic for ages 3+.  Details of the event are below:

Date: Saturday, October 7th from 10:00 AM - 1:00 PM

Where: Elmwood Park Zoo, 1661 Harding Blvd, Norristown, PA, 19401

 

Vaccine Offered:​​​

• ​Flu Shots - Ages 3+ (High Dose for ages 65+)

Walk-Ups Welcome - Pre-registration is highly recommended - Appointments Prioritized

• IF THIS IS FOR A MINOR, PLEASE REGISTER USING YOUR CHILD'S NAME, DOB, AND ALL OTHER INFORMATION!

If you are under 18, you must have either a parent/guardian present or have the proper consent form(s) printed and signed (NO EXCEPTIONS). ​​​

The insurance section of registration is not required but completion is highly encouraged.

Registration will close on October 4th.
