FLU/COVID VACCINE CLINIC 

In conjunction with Springfield SD, Eric's RX Shoppe is happy to partner to provide a vaccine clinic for ages 3+.  Details of the event are below:

Date: Monday, October 27th from 4:00 PM - 6:00 PM

Where: Enfield Elementary School - 300 Montgomery Ave  Erdenheim, PA 19038 - GYM

 

Vaccine Offered:​​​​

• Flu Vaccine - Ages 3-64

• Flu Vaccine - Ages 65+ (High Dose)

• SpikeVax by Moderna Covid 19 Vaccine - Ages 3-11​​

• mNEXSPIKE by Moderna Covid 19 Vaccine - Ages - 12+​

FOR ALL COVID VACCINES:
I understand the benefits of vaccination and possible side effects. I agree to receive the vaccine as part of a shared decision with my healthcare provider (pharmacist).
