In conjunction with TIFF Advisory Services, Eric's RX Shoppe is happy to partner to provide a vaccine clinic.  Details of the event are below:

Date: Wednesday, September 30th from 1:00pm-2:00pm​

Vaccine Offered:​​​​​​​​

• Flu Vaccine - Ages 3-64

• Flu HD Vaccine - Ages 65+ ​​

• mNEXSPIKE by Moderna Covid 19 Vaccine - Ages 12+​

FOR ALL COVID VACCINES:

I understand the benefits of vaccination and possible side effects. I agree to receive the vaccine as part of a shared decision with my healthcare provider (pharmacist).

Please pre-register and be sure to complete the insurance section of the registration. Uploading your insurance card will make it easier to verify coverage.

IF THIS IS FOR A MINOR, PLEASE REGISTER USING YOUR CHILD'S NAME, DOB, AND ALL OTHER INFORMATION!

It must be a minimum of 3 months since your last covid vaccine.  ​
