FLU VACCINE CLINIC 

In conjunction with TIFF Advisory Services, Eric's RX Shoppe is happy to partner to provide a vaccine clinic for ages 3+.  Details of the event are below:

Date: Wednesday, September 13th from 11:00 AM - 12:00 PM

Where: 170 N Radnor Chester Rd #300, Wayne, PA, 19087

Vaccine Offered:​​​

• ​Flu Shots - Ages 3+ (High Dose for ages 65+)

Pre-registration is highly recommended.

• IF THIS IS FOR A MINOR, PLEASE REGISTER USING YOUR CHILD'S NAME, DOB, AND ALL OTHER INFORMATION!

If you are under 18, you must have either a parent/guardian present or have the proper consent form(s) printed and signed (NO EXCEPTIONS). ​​

Please be sure to fill out the insurance section and/or upload a copy of your card. 

​

Registration will close on September 10th at 11am.
