In conjunction with the Upper Dublin School District, Eric's RX Shoppe is proud to provide a vaccine clinic

as a resource for the Upper Dublin School District community. Details of the event are below:

Where: Upper Dublin High School Cafeteria, 800 Loch Alsh Ave, Fort Washington, PA 19034

• Flu Vaccine - Ages 3-64

• Flu Vaccine - Ages 65+ (High Dose)

• SpikeVax by Moderna Covid 19 Vaccine - Ages 3-11​​

• mNEXSPIKE by Moderna Covid 19 Vaccine - Ages - 12+​

FOR ALL COVID VACCINES:
I understand the benefits of vaccination and possible side effects. I agree to receive the vaccine as part of a shared decision with my healthcare provider (pharmacist).
