501 COVID/FLU
VACCINE CLINIC 

In conjunction with The 501, Eric's RX Shoppe is happy to partner to provide a vaccine clinic for ages 3+.  Details of the event are below:

Date: Friday, September 20th from 9:30 AM - 11:30 AM

Where: 2815 Byberry Rd, Hatboro, PA 19040

​

Vaccine Offered:​​​

• ​Flu Shots - Ages 3+ (High Dose for ages 65+)

• Moderna SpikeVax (12+)

Pre-registration is highly recommended.

• IF THIS IS FOR A MINOR, PLEASE REGISTER USING YOUR CHILD'S NAME, DOB, AND ALL OTHER INFORMATION!

If you are under 18, you must have either a parent/guardian present or have the proper consent form(s) printed and signed (NO EXCEPTIONS).

The insurance section of registration is not required but completion is highly encouraged.
