REQUIRED VACCININATIONS FOR ENTRANCE 
GRADES 7th & 12th

In conjunction with Abington School District and The Montgomery County Office of Public Health, Eric's RX Shoppe is happy to partner to provide a vaccine clinic.  Details of the event are below:

Date: Wednesday, June 18th from 3:30pm-6:00pm

Where: Abington Senior High School, Cafeteria C- 900 Highland Ave, Abington, PA, 19001

 

Vaccine Requirements:​​

​FOR ATTENDANCE IN 7TH GRADE:

•    1 dose of tetanus, diphtheria, acellular pertussis (Tdap) on the first day of 7th grade.
•    1 dose of meningococcal conjugate vaccine (MCV) on the first day of 7th grade.

FOR ATTENDANCE IN 12TH GRADE:

• 1 dose of meningococcal conjugate vaccine (MCV) on the first day of 12th grade.

If one dose was given at 16 years of age or older, that shall count as the twelfth grade dose.

**Pre-registration is required. Insurance information must be included in the registration. If you do not have insurance, a dose will be provided by the county at no cost to you**

• IF THIS IS FOR A MINOR, PLEASE REGISTER USING YOUR CHILD'S NAME, DOB, AND ALL OTHER INFORMATION!

If you are under 18, you must have either a parent/guardian present or have the proper consent form(s) printed and signed (NO EXCEPTIONS).
