REP. SCOTT'S BOO AT THE ZOO
VACCINE CLINIC 

In conjunction with Rep. Greg Scott and Elmwood Park Zoo, Eric's RX Shoppe is happy to partner to provide a vaccine clinic for ages 3+.  Details of the event are below:

Date: Thursday, October 23rd from 5:00 PM - 7:00 PM

Where: Elmwood Park Zoo, 1661 Harding Blvd, Norristown, PA, 19401

 

Vaccine Offered:​​​​

• Flu Vaccine - Ages 3-64

• Flu Vaccine - Ages 65+ (High Dose)

• SpikeVax by Moderna Covid 19 Vaccine - Ages 3-11​​

• mNEXSPIKE by Moderna Covid 19 Vaccine - Ages - 12+​

FOR ALL COVID VACCINES:

I understand the benefits of vaccination and possible side effects. I agree to receive the vaccine as part of a shared decision with my healthcare provider (pharmacist).

Please pre-register and be sure to complete the insurance section of the registration. Uploading your insurance card will make it easier to verify coverage.
