VACCINE CLINIC

In conjunction with Community Partnership School, Eric's RX Shoppe is happy to partner to provide a vaccine clinic.  Details of the event are below:

Date: Tuesday, September 30th from 12:00 PM - 2:00 PM

Where: Community Partnership School-  3033 W Glenwood Ave, Philadelphia, PA 19121

 

Vaccine Offered:​​​

• ​Flu Vaccine - Ages 5-64

• Flu Vaccine - Ages 65+ (High Dose)

• SpikeVax by Moderna Covid 19 Vaccine - Ages 5-11

  • with at least one underlying medical condition that places you at higher risk for severe COVID-19 outcomes. Please see full list here​

• mNEXSPIKE by Moderna Covid 19 Vaccine - Ages - 12+

  • Have a previous history of receiving a COVID-19 vaccine (in order to receive mNEXSPIKE) AND

  • Age 65 and older OR

  • Age 12–64 with at least one underlying medical condition that places you at higher risk for severe COVID-19 outcomes. Please see full list here

Please be sure to pre-register and complete the insurance section of registration.

If you are under 18, you must have either a parent/guardian present or have the proper consent form(s) printed and signed (NO EXCEPTIONS).
