VACCINE CLINIC

In conjunction with Lower Merion School District, Eric's RX Shoppe is happy to partner to provide a vaccine clinic.  Details of the event are below:

Date: Friday, November 8th from 1:00 PM - 6:00 PM

Where: Lower Merion High School Gym - 315 E. Montgomery Ave, Ardmore, PA, 19003

 

Vaccine Offered:​​​

• ​Flu Shots - Ages 3+ (High Dose for ages 65+)

• Covid-19 Vaccine (Moderna ONLY)

Pre-registration is highly recommended.  Be sure to complete the insurance section of registration.

• ​IF THIS IS FOR A MINOR, PLEASE REGISTER USING YOUR CHILD'S NAME, DOB, AND ALL OTHER INFORMATION!

If you are under 18, you must have either a parent/guardian present or have the proper consent form(s) printed and signed (NO EXCEPTIONS).
