In conjunction with the Senator Collett, Eric's RX Shoppe is proud to provide a vaccine clinic.

Details of the event are below:
 

​Date: Friday, May 8th from 10:00 AM - 12:00 PM 
 

​​Where: Montgomery Township Community & Recreation Center       
 

​Vaccine Offered:​​​​​​​

• 2nd Dose Covid Vaccine - mNEXSPIKE by Moderna ​

  • Must be a 65+ years of age and a minimum of 6 months from your last covid vaccine,  OR

  • Immunocompromised and minimum of 2 months since your last covid vaccine

• Shingles​

• RSV

• Pneumonia

​FOR ALL COVID VACCINES:

I understand the benefits of vaccination and possible side effects. I agree to receive the vaccine as part of a shared decision with my healthcare provider (pharmacist).

Please be sure to pre-register and complete the insurance section of registration.  Registration is required for Shingles, RSV and Pneumonia vaccines.  We must verify insurance coverage.​​
